<p>Responds to statewide requests for CHR services. Provides clinical screening & triage to multiple levels of care. Refers individuals and/or families to the appropriate level of care in or out of our service system. Crisis triage to 911/emergency services and/or MCRT & EMPS (via 211) as needed. Triage Center is staffed by a team of call center agents, two care coordinators, an access coordinator and a manager.</p>

<p><strong>Duties & Responsibilities</strong></p>
<ul>
<li>Participate as member of multi-disciplinary treatment team</li>
<li>Track and coordinate client intake, discharge, services and referrals as applicable</li>
<li>Provide assistance to team including coordination of treatment planning and providing daily triage.</li>
<li>Educate and link individuals to self-help groups, natural support and other resources as needed.</li>
<li>Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payee, legal services, medical services, education and employment.</li>
<li>Maintain careful documentation of contacts and communication to maintain quality services. Complete all required documentation in compliance with agency policy.</li>
<li>Work as a liaison between agency and other relevant providers and teams to coordinate care for referred clients.</li>
<li>Assist with scheduling and providing transportation to and from appointments.</li>
<li>Maintains documentation in compliance with agency and program standards.</li>
<li>Participates in staff meetings, supervision, training and other meetings as assigned.</li>
<li>Establish knowledge of cultures and communication styles associated with various groups, particularly oppressed groups, to provide culturally sensitive and competent care.</li>

<p><strong>Education:</strong> Bachelor’s degree in human service-related field required. Experience may be accepted in lieu of education for some positions.</p>
<p><strong>Experience:</strong> A minimum of two years’ related experience preferred.</p>
<p><strong> Licensure/Certification/Registration:</strong> Valid driver’s license required. </p>

<p>$19.2850/hr min, and up commensurate upon experience.</p>
<p>*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)</p>
