<p>East Hartford Outpatient (EHOP) therapists have the unique opportunity to provide therapy to adults, children and families. Therapists see clients in person and through telehealth for diagnostic assessments, treatment planning, individual and family therapy sessions. Opportunities for case consultation, supervision, training and professional growth are provided.</p>
<p><strong><span style="text-decoration: underline;">ABOUT THE POSITION:</span> Care Coordinator</strong></p>
<p>Coordinate and monitor the implementation of plans that address the behavioral health needs of the client. To work in partnership with the client and treatment team to coordinate and triage daily tasks and care for clients. Communicate and function as a liaison between services and providers. Complete any necessary reporting in a timely manner. Advocate and support clients to address treatment barriers such as access, transportation, etc. in order to increase the independence and self-efficacy for individuals and families within our services. </p>
<p><strong>Duties & Responsibilities:</strong></p>
<ul>
<li>Participate as member of multi-disciplinary treatment team</li>
<li>Track and coordinate client intake, discharge, services and referrals as applicable</li>
<li>Provide assistance to team including coordination of treatment planning and providing daily triage.</li>
<li>Educate and link individuals to self-help groups, natural support and other resources as needed.</li>
<li>Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payee, legal services, medical services, education and employment.</li>
<li>Maintain careful documentation of contacts and communication to maintain quality services. Complete all required documentation in compliance with agency policy.</li>
<li>Work as a liaison between agency and other relevant providers and teams to coordinate care for referred clients.</li>
<li>Assist with scheduling and providing transportation to and from appointments.</li>
<li>Maintains documentation in compliance with agency and program standards.</li>
<li>Participates in staff meetings, supervision, training and other meetings as assigned.</li>
<li>Establish knowledge of cultures and communication styles associated with various groups, particularly oppressed groups, to provide culturally sensitive and competent care.</li>
<li>Provides crisis intervention and support as clinically indicated.</li>
<li>Additional duties as required.</li>
</ul>
<p><span style="text-decoration: underline;"><strong>QUALIFICATIONS</strong><strong>: </strong></span></p>
<p><strong>Education:</strong> Bachelor’s degree in human service-related field required. Experience may be accepted in lieu of education for some positions.</p>
<p><strong>Experience:</strong> A minimum of two years’ related experience preferred.</p>
<p><strong>Licensure/Certification/Registration: </strong>Valid driver’s license required.</p>
<p><strong>Training: </strong>Complete any program specific training required. Complete required annual agency trainings. CPR/First-Aid Certification may be required within 6 months of hire</p>

<p>*Actual rates are determined at the time of offer and are based off relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)</p>
